Details transfer over from Questionnaires or other forms

Hi there, it would be extremely helpful if when patients who are filling out questionnaires, the details they enter such as their date of birth etc are transferred and entered over automatically to the patient card. It would save a lot of time as opposed to having to enter it manually each time.
