I have noticed that when there is a blockout in the diary it is still possible to book patients in to that time which is blocked out and the blockout is not clear. This is obviously going to lead to some double bookings.

Have you checked whether you allow overlapping appointments in your calendar settings? You can find it in setup > settings > calendar settings.
