Is there a way to collect data from client medical forms? For example, running a report for all patients who have noted they currently smoke or all patients who have certain medical conditions as noted in intake and/or pre-operative assessment forms?
Ideally there would also be a report template that would be Admitted Patient Care data sets for PHIN reporting.

Hi.
Yes, but, for the time being, only through the linked fields feature, which is a question of a medical form being linked to a custom field.
Then you would run a client report and see the input in those custom fields, for every client. Do note however, that this approach doesn’t work for questions applicable to temporary events, and that the new input removes the old input in the custom field.
